HIPAA

According to the Health Insurance Portability and Accountability Act of 1996 (HIPAA), all new and existing patients will be given the opportunity to read the Notice of Health Information Practices at check-in, and required to provide their signature of having either read it or been given the opportunity to read it.

Take home booklets are available upon request or you may print it or download it now.

Medical Records Charges

The fee charged by Family Healthcare • Newberry for the copying, supplies, labor, and duplication of medical records is $15.00 for the first 30 pages or $0.65 per page, whichever is the lesser, and $0.50 for each additional page, in addition to any postage costs to the practice. Charges apply to all patients requesting records, all insurance companies, SC Vocational Rehabilitation, and all attorney offices requesting medical records. Payment is due before the records will be released.

Please note that all records released must be pre-authorized by either the patient (at least 18 years of age) or a parent/legal guardian. A records release form can be obtained at Family Healthcare • Newberry.

All requests for NFHC to complete insurance/workers compensation forms will be $10.00 for the 1st page and $5.00 for each additional page thereafter. Payment is due before the forms will be filled out.
